Requirements for Employees
New Groups Enrolling in Medical Plans
- Completed original enrollment form.
- Student Verification for any dependent who is a full time student between the ages of 19 and 23.
- If you're part of a new group enrolling into a health plan through the AIA and you are waiving the coverage, complete the Small Employer Health Benefits Waiver of Coverage form.
New Groups Enrolling in Dental Plans
- Completed United Concordia enrollment form
To change plans as an existing group medical plan through the AIA
- Completed Employee Enrollment Form
