Enrollment

To be eligible to enroll in the Philadelphia Chapter AIA health plans, a firm must have at least two employees. The two employees can enroll in different health plans, one being in Keystone and the other in Personal Choice. Individuals who are licensed architects may join as a one-person group if you can prove that you are earning a living as an architect by providing a copy of your Schedule C.

To request a quote for medical insurance, please fill out the Rate Request Form. After clicking on Submit, your request will automatically be sent to Gallagher Benefit Services, Inc.

Each new group interested in enrolling is required to complete ORIGINAL forms for the employer group as well as the individual employees to be covered. Please refer to the Requirements for Employers section for a complete listing of what is needed to enroll in a group medical plan. Please call Gallagher Benefit Services, Inc. to have the original forms mailed to you. Please make sure that all items are fully completed when returning your application. Gallagher Benefit Services, Inc. must receive all forms no later than 20 days prior to the effective date of the coverage. Otherwise, the group will be delayed for another month before coverage becomes effective.

Each year at open enrollment, employees must be given the option of changing health plans, if more than one plan is offered.
