HSA Qualified High Deductible Health Plans
High Deductible Health Plans can be paired with Health Savings Accounts (HSA). HSA's are member owned personal savings accounts, which can be funded, by either the employer or member. HSA's provide a tax-free savings vehicle for medical expenses, structured similar to an IRA. Qualified deductions from the HSA are tax-free while non-qualified deductions are subject to tax and penalty. Unused funds can accumulate year after year to pay for future medical expenses to supplement retirement savings. The member owns the account so if they change jobs, the account goes with them.
Personal Choice
Personal Choice gives you the freedom of choice by allowing you to choose your own doctors and hospitals without the necessity of a primary care physician or referrals. Maximum coverage is available by accessing preferred providers within the Personal Choice network. If you receive services from out-of-network providers, you will have higher out-of-pocket costs due to deductibles and co-insurance.

Physician Visit Co-pays
- $20 Preventive Visits (Pediatric/Adult) and Routine GYN Exam/PAP (1 per year)
NOTE: The above services do not require that you meet the deductible before paying the co-pay amount.
Annual Deductible for Office Visits, Outpatient Services, Facility and Ancillary Services
- $1,500 Single / $3,000 Family
- $2,000 Single / $4,000 Family
- $2,500 Single / $5,000 Family
- $3,000 Single / $6,000 Family
Coinsurance for Office Visits, Outpatient Services, Facility and Ancillary Services
- 0% to annual out of pocket maximums set by IRS
- 20% to annual out of pocket maximum set by IRS
2009 Out of Pocket Maximums are $5,800 Single / $11,600 Family
NOTE: Members are not required to meet the deductible before paying the coinsurance amount for mammograms and pediatric immunizations.
OPTIONAL Prescription Drug Benefit
- $5 Generic Formulary - $20 Brand Name Formulary - $45 Non Formulary
Members can get a 30 day supply of medications at a retail pharmacy. After reaching the medical plan's deductible, members will be responsible for paying a copay in network. Out of network, members are responsible for the applicable coinsurance.
