Dental FAQ

Q. What dental plans are available to member firms?

A. There are two United Concordia dental plans available — Basic & Comprehensive. Additionally, there are two Keystone dental rider plans available to those firms/members enrolled in a Keystone HMO or POS plan.

Q. We have health insurance through another carrier or we already have the medical insurance through the Cultural Alliance. Can we add just Dental to our plan?

A. Yes, you may add a Dental Plan. You can choose one of the United Concordia dental plans for your entire group. If your group has less than 10 employees, 100% employee participation is required. Otherwise you must have 75% employee participation.

Q. I am a sole practitioner with no employees. How can I acquire coverage for myself (and my family)?

A. Unfortunately, the dental coverage is only available to those companies who have at least 2 employees and are insuring at least 2 employees.

Q. Do employees need to select a dentist at the time of enrollment?

A. No, employees may choose any licensed dentist for services. If the dentist does not participate in the United Concordia network, he/she may balance bill the member the difference between what United Concordia pays and their charge. Participating dentists agree to accept the Maximum Allowable Charge as payment in full for covered services.

Q. Will employees have to submit their own claim forms?

A. When using a non-participating dentist, employees may have to pay the dentist at the time of service, then submit their own claims for reimbursement. Participating dentists will submit the claims for you. The United Concordia dental claim form is #13 on this website's forms page.

Q. Up to what ages are dependents covered?

A. Dependents are covered up to age 19 or 23, if enrolled full time as a student in an accredited educational institution. Proof of full-time student status must be furnished to the carrier within 30 days of attaining the limiting age. Proof will need to be resubmitted on an annual basis. Q. How do we enroll in the plan?

A. Complete the United Concordia Group Enrollment Form (#10 on the forms page of this website) and have the enrolling employees complete the United Concordia Enrollment Form (#11 on the forms page of this website). These forms can be faxed to Kristen Garry at 215.884.8055. You do not need to submit a check for the first month's premium. You will receive an invoice directly from United Concordia each month.

Q. How do I notify United Concordia of employee changes, additions or terminations?

A. Complete the United Concordia Enrollment Form (#11 on the forms page of this website) and fax it to United Concordia at 800.329.9093. Otherwise, you can send a request to Kristen Garry via e-mail or fax 215.884.8055. Be sure to include the employee name, ID # and effective date of termination.

Q. How can I submit employee changes, additions or terminations electronically?

A. If you are already enrolled with Group Query, you can log into it at www.ucci.com. If you need to register as a first time user, call your membership and billing representative at the number listed at the top of your invoice.

Q. When can I make enrollment changes such as adding a dependent?

A. After initial enrollment, there are certain life events that permit you to add dependents - birth, adoption, court order of placement or custody, change in student status, marriage. If one of these events occur and you need to add a dependent onto your dental plan, you must notify your group and supply the required enrollment change information within 31 days of the date of the event.

Additionally, enrollment changes can be made without a qualifying life event at each open enrollment period.
